The California Community Transitions Program helps people living in skilled nursing facilities move back to their own home or into an assisted living setting.
All Hours Adult Care is a state-approved Care Transition Coordinator. Our team provides the support and resources needed to make the move home as smooth as possible.
Assistance with finding and securing affordable housing, including rent assistance
Personal care and assistance with daily living activities
Coordinating home health care
Help with setting up a home with home modifications, furniture, and assistive devices

The California Community Transitions (CCT) program is a Medi-Cal-funded service that helps people who have lived in a licensed healthcare facility for at least 60 days return to their home or another community setting. Run by the state Department of Health Care Services (DHCS), the program gives Medi-Cal beneficiaries the extra support they need to leave a medical facility and live more independently and safely.

If you have Medi-Cal and have been living in a hospital, nursing home, or other licensed medical facility for at least 60 days, you may qualify for the CCT program. It’s designed to help people who still need some support, like the care they received in the facility, move back into their own homes or a community setting.
• There’s no age limit for the program
• You must have Medi-Cal or be eligible for it
• You must have lived in a licensed facility (like a skilled nursing facility) for 60+ days
• You must want and need help to live independently after leaving the facility
• Short-term rehab stays paid for by Medicare don’t count toward the 60 days -
To get started, you’ll first contact a local CCT Lead Organization. All Hours Adult Care is a CCT Lead Organization in Riverside, San Bernardino, Orange and Los Angeles Counties. You can find a complete list of approved CCT Lead Organizations on the California Department of Health Care Services website (https://www.dhcs.ca.gov) or contact All Hours Adult Care for our service areas.
Once you connect with the lead organization, a CCT coordinator will meet with you to talk about your needs and help you create a plan for moving out of the facility where you currently live. They’ll help you transition to a place in the community that works for you—whether that’s your own home, a family member’s home, or an apartment.

The CCT program provides a variety of helpful services to make your move from a facility to your home or community as smooth as possible. Here’s what you can expect:
A personal assessment to understand your needs and create a transition plan
Help finding and securing housing that fits your situation
Home modifications like ramps or grab bars to make your new space safe
Assistance with getting medical equipment and supplies you’ll need at home
Support with setting up utilities and other essential services
Training in self-care and daily living tasks so you feel confident living independently
Coordination with doctors, caregivers, and local support services
Ongoing follow-up and support for up to one year after you move

You’ll receive support for up to 12 months (365 days) after you move into your new home or community setting. During that time, the CCT team will continue to check in, help you adjust, and make sure you have access to the services you need to live safely and comfortably.

The program assists in identifying suitable housing options and coordinating the move. Participants can transition to various community settings, including:
• Independent living in a private home or apartment
• Assisted living facilities
• Other community-based residential settings that meet the individual's need -
