Do you need to find your local county APS office? CLICK HERE to search the list.

To Apply:

  1. Submit an active APS case referral on county letterhead

  2. Return APS / Community Intake Packet. (Current wait time minimum 7 months)

  3. Return RCFE (LIC602A) from your primary care physician.

  4. If currently residing in an assisted living facility, return the LIC603 document.

  5. Submit if Applicable:

    Designated Power of Attorney (DPOA)

    Advanced Healthcare Directive (AHCD)

    Proof of State Conservatorship
