Do you have an active Adult Protective Services case?

NOTE: You MUST send an APS letter to our office to be placed on the correct APS waitlist.

Who is eligible?

  • Individuals with an active adult protective service case (must provide a formal referral from your case worker on county letterhead)

  • Individuals with active Medi-Cal

  • Individuals with the ability to pay rental fees

  • Individuals who can either make their own decisions or have a written authorized representative to make decisions on their behalf

Steps to Apply:

  1. Complete the required Community Intake Packet.

  2. Return the documents to our office via email or fax.

  3. Get a recommendation for RCFE (LIC602A) from your primary care physician.

  4. Instruct your APS case worker to send a letter to our office.*
    •Letter must be on county letterhead stating there is an open case with the case number.

  5. Once received, you will be placed on the APS waitlist
    Current wait time: MAX 30 days

    1. If the resident is currently a resident at an assisted living waiver, download the LIC603 document and provide it to the facility to be completed.
